Your Social Prescribing Journey
Thank you for your referral to the Jurassic Coast PCN Social Prescribing Team. Social prescribing is not an urgent service, and during busy periods there may be a wait of a few weeks before a member of the team is able to contact you.
In the meantime, we’ve put together some helpful information and local resources you can access while you wait. If you have a question about your referral or would like an update, feel free to call us on 01308 428943 or contact us via secure online form.
