Thank you for your referral to the Jurassic Coast PCN Social Prescribing Team. Social prescribing is not an urgent service, and during busy periods there may be a short wait before a member of the team is able to contact you.

In the meantime, we’ve put together some helpful information and local resources you can access while you wait. If you have a question about your referral or would like an update, feel free to call us on 01308 428943 or contact us via secure online form.

What is Social Prescribing

Social Prescribing connects you with practical support, activities, and services in your community to improve your wellbeing. This might include:

  • Support with loneliness, housing, or finances
  • Help to be more active or manage long-term conditions
  • Links to creative activities, community groups, or volunteering
  • Emotional and mental wellbeing support

Our team takes time to understand what matters to you and works with you to find the right support.

For adults

If you need urgent support for your mental health, you can contact Connection, a 24/7 mental health advice line:

There is crisis mental health support at The Community Front Room in Bridport from Thursday to Sunday – 2:15pm to 9:45pm:

You can also self-refer to Steps 2 Wellbeing, the NHS provision for talking therapy in Dorset:

For support with moving more, managing weight, stopping smoking, or drinking less you can self-refer to LiveWell Dorset:
