Your Social Prescribing Journey
Thank you for your referral to the Jurassic Coast PCN Social Prescribing Team. Social prescribing is not an urgent service, and during busy periods there may be a short wait before a member of the team is able to contact you.

What is Social Prescribing
Social Prescribing connects you with practical support, activities, and services in your community to improve your wellbeing. This might include:
- Support with loneliness, housing, or finances
- Help to be more active or manage long-term conditions
- Links to creative activities, community groups, or volunteering
- Emotional and mental wellbeing support
Our team takes time to understand what matters to you and works with you to find the right support.
